Learn about how to receive your disability placard to apply for disabled parking.
1. Obtaining a Disability Parking Placard
You must first complete a Persons with Disabilities Certification for Parking Placard/License Plates form (VSD 62), and have it certified by a licensed physician that can assess your condition.
Once completed, the form must be mailed to the Secretary of State's office by you or your physician on your behalf.
When the application gets approved, you can apply for disabled parking.
2. Qualifications
Disabled individuals who apply for a restricted parking space sign must meet the following:

The applicant must reside on a residential street that is zoned R-1 through R-5.

The applicant must have either a current disabled Illinois license plate or a disabled placard issued by the Illinois Secretary of State.

The applicant must reside at the location for which the signs are being requested.

The applicant may not have access to off-street parking.

The number of restricted parking spaces on the street does not exceed the maximum allowed on a residential street.
3. Applying for a Disabled Parking Sign
Fill out this form to apply for a disabled parking sign. There is a $70.00 fee associated with this service, and a $25.00 annual maintenance fee for the sign.
